Gerald A. Prevatt, 36, of 16547 Route 178, Adams, is facing four charges following a state police investigation into an incident in which a 20-year-old woman was provided alcoholic beverages in the town of LeRay.
He is summoned to appear in LeRay Town Court for arraignment on charges of unlawfully dealing with a child, making a false written statement, second-degree aggravated unlicensed operation of a vehicle, and using a different vehicle to avoid an interlock device.
He was charged Sunday at the Metro-Jefferson Public Safety Building, where he was sent Friday for violating probation.
Mr. Prevatt was sentenced in February to five years probation and fined $2,500 after he pleaded guilty to three counts of driving while intoxicated, two of those being felonies, in satisfaction of arrests occurring from October 2009 to May 2010.
The underage-drinking investigation was opened Sept. 25.
