Heather L. Carr, a licensed practical nurse, agreed to an indefinite license suspension until shes fit to practice again, after she admitted she created fraudulent prescriptions, according to a state Education Department and Board of Regents professional misconduct report.
According to the report, which was finalized in November and recently made available on the departments website, Ms. Carr also agreed to two years probation, after her suspension, before she can return to practice. She also must pay a $500 fine.
Ms. Carr admitted to state police last year she forged prescriptions to two pharmacies to support her dependence on Vicodin. She also sold about 80 pills for $80. She was fired May 30 from Samaritan Medical Center, where she had been employed for six years.
